Social ServicesTopics you will find on this page: Inpatient ServicesSocial workers assess and follow members who are hospitalized at both Kaiser Permanente's San Diego Medical Center and Zion hospital, based on referrals and high-risk screening. They assist and counsel members regarding many issues including those related to severe illness, death and dying, drug and alcohol-related issues, adoption, social needs, financial needs and resources, and ethical dilemmas. These services are available as a benefit to every individual in the hospital. A message can be left for a social worker at: (619) 641-4456. Social Services Support in Clinic SettingsIn addition to their hospital, research or community responsibilities, Continuing Care Services social workers also provide counseling, support and social work assistance to members being seen in a number of outpatient clinics. These include: Members may be referred to one of these clinics through various sources. Critical Incident Response TeamContinuing Care social workers also provide assistance to Kaiser Permanente members and staff through the Critical Incident Response Team. This team is available for counseling staff during and after crisis situations such as the Cedar Fires. Family Violence ProgramA Continuing Care Services social worker acts as the liason for the Family Violence Program for Kaiser Permanente physicians, employees and the community. Mandated reports are collected from Kaiser Permanente staff concerning Adult/Elder Abuse, Domestic Violence and Child Abuse, and annual statistics are compiled related to these reports. This liaison acts as a resource by answering questions regarding reporting laws and giving educational presentations about these laws and the reports. Abuse Resources. |
